D&E Communications, Inc. and its subsidiaries recognize that a total compensation philosophy is key to our continued success. That’s why we provide a comprehensive employee benefits package. Coverage is based on Regular Full-Time employment (35 hours or more per week) unless otherwise noted in the Summary Plan Description.

Health Care Benefits 2008

Benefits are effective the 1st of the month following 60 days of active employment.

Medical/RX

Medical coverage is provided through Highmark Blue Shield PPO. There are three (3) program levels (High, Core, Basic) to choose from. The programs offer employees flexibility and choices in different limits of deductible, co-payments, coinsurance, and the amount of employee premium share through pre-tax payroll deduction.

Prescription drug coverage is provided through Medco. There are three (3) tier levels of co-payment for generic, brand formulary, and brand non-formulary prescription drugs. You can receive up to a 31-day supply at a retail pharmacy, or use the mail order service for a 90-day supply.

Vision Program

Vision coverage is provided through Davis Vision. There is a level of benefit for both in-network and out-of-network services for eye examinations and eyeglasses (frames/lenses or contacts).

Dental Program

Dental coverage is provided through Sun Life (DenteMax Network). You may visit any dentist you wish and receive a specific limit of reimbursement. Coverage is provided for preventive, basic, and major dental services, plus a limited orthodontic benefit for children under age 19.

Flexible Spending Accounts (FSA)

A Flexible Spending Account is a voluntary program that allows you to convert part of your compensation into tax-free benefits. Contributions made through your FSA are free of Federal and FICA taxes. Under your FSA program, you may elect to have a portion of your wages deducted before taxes, to use for payment of health care and/or daycare expenses.

Other Welfare Benefits

Short-Term Disability (STD) Benefit

Short-Term Disability benefits are payable after an elimination period of seven (7) continuous calendar days, benefits start on the eighth day. Employees receive 67% of their basic weekly earnings for up to 25 weeks. Commissions are averaged for sales positions. Benefit is effective the first of the month after 90 days of active employment.

Long-Term Disability (LTD) Insurance

Long-Term Disability benefits commence after a 180-day elimination period. Employees receive 60% of basic wages, up to $10,000 monthly maximum. Commissions are averaged for sales positions. Benefit is effective the first of the month after 90 days of active employment.

Group Term Life Insurance and Accidental Death & Dismemberment (AD&D)

Employees receive $75,000 Term Life Insurance and $75,000 Accidental Death and Dismemberment. Benefit is effective the first of the month after 90 days of active employment.

Voluntary Term Life

Employees may elect to purchase additional life insurance coverage for themselves and their dependents through convenient (after tax) payroll deductions. Benefit is effective the first of the month after 90 days of active employment.

Employee Assistance Program (EAP)

The Employee Assistance Program provides confidential assessment, counseling, and referral services for eligible employees and their dependents. Up to five (5) free sessions per calendar year are available. Benefit is effective on the first day of employment.

Retirement Benefits

401(k) Savings Plan

The 401(k) Savings Plan is designed to give eligible employees the opportunity to save for retirement while deferring the payment of federal tax on those savings until withdrawn. Savings through this plan will reduce your current federal income taxes because your contributions are made before federal tax is withheld.

Participants may contribute 1 to 60% of their eligible wages (pre tax) immediately upon their date of hire, and they will become eligible to receive an Employer Matching Contribution beginning with the first pay date of the month after completing six months. Employees become fully vested in the Employer match after 5 years. You may select the Plan's investment options that best fit your needs and savings goals. Refer to the Summary Plan Description for further details.

Time Off Policies

Holidays (6)

New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day. Employees are eligible for holiday pay after the first day of hire.

Paid Time Off (PTO)

Prorated in first year of hire as follows: 1st calendar year - For each eligible week of the initial calendar year of employment, the new employee shall accrue 1/52 of 80 PTO hours (ex. 1/52 X 80 = 1.5385 hrs. per week, times the number of weeks worked, rounded to the nearest hour for non-exempt and nearest whole day for exempt). Employees hired after November 30th are not eligible for PTO until the beginning of the new calendar year at which time they are considered to be in their 2nd year of employment for the purpose of PTO calculation. Employees are eligible after 30 days of continued active employment.

Additional weeks of Paid Time Off are accrued as follows:

*Prorated for full-time employees who work fewer than 40 hours per week.

Sick Days

Up to three (3) days earned per year (prorated first year); maximum accumulation 200 hours. Employees are eligible after 30 days of continued active employment.

Bereavement

Employees are eligible for up to three (3) days upon the death of an immediate family member-see handbook for definition of immediate family members. Employees are eligible after 30 days of continuous active employment.

Jury Duty

Eligible employees will be compensated for the difference between jury duty pay and the employee's basic wages.

Additional Benefits

Credit Unions

Belco Community Credit Union and Service 1st Federal Credit Union. Employees are eligible on their first day of employment.

Group Discount Insurance

Donegal Mutual Insurance offers various discounts for personal insurance such as home, auto, etc. Employees are eligible their first day of employment.

Consern Loans

Financial assistance for educational expenses is available through Consern Loans.

Job Enrichment Educational Assistance Program

Employees are eligible to complete an application for participation in the education assistance program, after completing the introductory period, usually six (6) months. Reimbursement will be up to 75% for employees who qualify for this program.

Employee Referral Program

You may be eligible for a $250 reward if you refer an applicant for an open position and the individual is hired. Contact an HR Representative for more information or visit the Intranet for an Employee Referral Form.

Employee Perks Program

After 30 days of employment, you may be eligible for free services such as Local Phone Service, Subscriber Line Charge and a number of our most popular Custom Calling Features, as well as Jazzd Dial Up and DSL, depending on the location of your residence. You may qualify for Expanded Basic Cable Service or Essentials Digital TV Service if you are within the service area. Refer to the Employee Perks Form located on the Intranet under "Forms, Customer Service, Brownstown"for details.

Stock Purchase Plan

Employees who generally work greater than 20 hours per week are eligible to participate after six months of employment. Participants may elect to have 1-10% of their base wages (after tax) withheld for the purpose of purchasing company stock through payroll deduction, at a five percent (5%) discount.
